Upload a custom consent form that participants must agree to before participating.
Custom consent forms are configurable by account administrators only. No other roles can configure this feature.
To upload a consent form, go to Account Settings > Template Library.
On the Consent Templates tab, click Upload consent form.
On the Upload a consent form screen, you can upload and name your consent form.
Once that's uploaded, you can view & delete the forms.
To mandate that all team members add consent forms to their project, turn this toggle on:
Once that's toggled on, team members will not be able to launch projects without selecting a consent form.
When setting up a project, you'll see all of your consent forms on the Details page.
If consent forms are mandatory in your workspace, you'll see this message if you haven't selected a form yet.
